2025/2026 Required Forms
Child and Youth Participation Form
This form must be completed annually, by a parent or guardian, for everyone under the age of 18 who will be participating in any activity at Holy Trinity. Please complete one form per child.
Volunteer Forms
Not sure which form to fill out? No worries, we've got you! If you're NEW to volunteering, B-1 is your go-to. If you're a seasoned pro and RETURNING for another round of fun, B-2 is for you. And, just to keep things interesting, everyone gets to fill out C (because, hey, who doesn't love a little paperwork?)!
